David C. Blomquist, LCSW , President
Blomquist Hale Consulting Group
is a Licensed Clinical Social Worker in the state of Utah. He graduated with a Masters Degree from the University of Utah in 1978. His program emphasized alcohol and drug dependency. He is a widely recognized pioneer in the field of Employee Assistance in the state of Utah and has served as a consultant to the governor of Utah on several occasions.

   
Rich Jensen, CPC, Director of Ogden Office
Graduated from the University of Phoenix with a Masters of Mental Health Counseling. He has a BS degree in Psychology and has extensive experience working in psychiatric facilities and with family services. He excels in working with families and couples as well as substance abuse.
   
Cortney Henrie, LCSW
Received a Masters Degree from the University of Utah in 1982. He has 20 years of experience in substance abuse. Areas of expertise are families, couples, abuse issues, adjustments to tragedies, sexuality, depression and anger management.
   
R. Mark Robertson, CSW
Graduated with a Masters Degree in Social Work. He has worked many years with family, child and adolescent issues. In addition, Mark excels in working with couples, anxiety and depression.
   
Teresa Ann Burton, LPC, CAC
Is a Licensed Professional Counselor and Substance Abuse Counselor. Graduated with a Masters in Mental Health Counseling from the University of Phoenix. Has extensive background in working with substance abuse and addiction. Works well with family issues, marital and personal issues.
   


Tracy Brockhaus, MFT
Graduated from the University of Phoenix with a Masters Degree in Counseling/Marriage Family and Child Therapy. Her expertise includes individual, family, child and adolescent issues. She also enjoys working with marital, parenting and personal issues.

   

Julianne Koer, LCSW
Graduated from New Mexico Highlands University with a Masters Degree in Social work and is licensed as a Clinical Social Worker in the State of Utah. She has worked many years in mental health and hospital settings. She has a broad knowledge of community resources. Her expertise includes geriatric/aging, parenting, marital issues, working with youth and crisis work. She enjoys counseling individuals, couples and families.
Michelle Slaughter, Office Manager
Michelle is the office manager in our Ogden office. She has worked for Blomquist Hale Consulting since 1988 and has been an integral part of it's growth and success. She is the first contact people make with calling the Ogden office. She has excellent people skills and has unparalleled skills in customer service and problem solving. Her focus on customer service readily puts people at ease. She is proud to work and be a part of a team that delivers services with integrity and dedication to overall service.
