Darren Elkins, M.S., LMFT, SAP
Salt Lake City Director

Darren received his Masters degree in Marriage and Family Therapy from Utah State University. He also holds two Bachelors Degrees from the University of Utah in Psychology and Family & Human Development. He served on the Utah Association of Marriage and Family Therapy board. Darren has a wide range of experience in workplace issues, leadership training, marital therapy, assessment and referral for drug and alcohol services, presented at various conferences, served on the Utah Association of Marriage and Family Therapists Board. Since joining Blomquist Hale in 2008, Darren has helped numerous individuals, employees, leaders, and families discover how to incorporate their strengths towards solutions for their ongoing problems. He enjoys working with couples and families, as well as individuals seeking to understand and find help with addiction. Darren enjoys the outdoors, traveling when he can, and spending time with his family.

Adam Munk, Ph.D., LMFT, SAP
Hablo Español

Adam received his PhD in Marriage and Family Therapy from Texas Tech University. He also received a Masters Degree in Marriage and Family Therapy from Utah State University. Additionally, he received his Bachelors Degree in Family Science, with a Minor in Psychology, from Brigham Young University. His professional experience includes providing individual, couple, family, and group therapy in Spanish as well as English and providing supervision to therapists seeking licensure. He is on the Department Of Professional Licensing board for Licensed Marriage and Family Therapists. Further experience includes curriculum development and instructing of leadership trainings, wellness workshops, group therapy, and team building. He teaches continuing education on ethics and other topics for Utah’s Association for Marriage and Family Therapists. Additionally, he has experience instructing college courses as Texas Tech University (Understanding Alcohol, Drugs, and Addictive Behaviors) as well as Utah State University (Marriage and Family Relations; Family and Cultural Diversity). He also has multiple professional publications and presentations at state and national conferences. When not at work, Adam is a member of a rock-n-roll band, playing the drums, guitar, and singing. He also enjoy playing disc golf with his wife, kids, and friends.

Since starting with Blomquist Hale in 2010, Adam has enjoyed working with people on a great variety of topics and believes the world is ever-changing, with new concerns and issues to be addressed all the time. He enjoys working with clients to find the helpful patterns and processes that help to overcome the problematic ones that occur in life. He recognizes that one size does not fit all and works with clients to change processes, behaviors, and thought patterns in ways that fit best for their goals.

Adriane Andersen, CMHC 

Adriane Andersen graduated from Westminster College with a degree in Professional Counseling and has been working in the clinical field for over four years. Adriane is passionate about her work and finds tremendous fulfillment in helping others through this challenging life. Licensed as a Clinical Mental Health Counselor, her experience includes substance abuse/addiction, marriage and family issues, anxiety/depression, eating disorders, and interpersonal conflict.  Adriane particularly enjoys working with adolescents, couples, and adults, and approaches therapy collaboratively, believing that both therapist and client bring important expertise and wisdom to the process. Adriane works alongside her clients, empowering them to create the desired change in their life with therapeutic support and guidance.  Acceptance and Commitment Therapy, Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, resources from the Gottman Institute, and Buddhist philosophies are all tools that Adriane regularly utilizes in her approach. In her free time, she enjoys being out in nature, being with family and friends, searching for the bottom of the internet, or the occasional indulgence of a good Netflix marathon.

Brad Hammel, LCSW, SAP

Brad has worked for over 16 years as a professional counselor and has extensive experience in substance abuse prevention, treatment and assessments. He is certified as a Substance Abuse Professional (SAP) and able to perform evaluations for individuals who fall under DOT regulations. He is licensed as a LCSW. Brad graduated from the University of Utah's Alcohol and Drug Training Program in 1997. He graduated from the University of Utah with a Bachelor of Science in Health Education in 1998, then from the U of U Graduate School of Social Work in 2002. He specializes in Alcohol & Drug evaluations and treatment. Brad works well with individuals who have anger issues, anxiety, depression, internet addiction, and other behavioral addictions. He is versed in Cognitive Behavioral therapy and Motivational Interviewing to help with these issues. Brad has been working at Blomquist Hale Consulting since 2004.

Brock Yancey, M.S., LCMHC

Brock received his Masters Degree in Counseling Psychology from the University of Utah in 1983 and is licensed as a Clinical Mental Health Counselor. Since 1990, Brock has worked with the Salt Lake County Division of Youth Services, currently as a manager for Counseling Services. Brock has been with Blomquist Hale since 1998 and has a broad range of experience working with children, adolescents, individuals, couples and families. He enjoys teaching mindfulness and believe that we all have the answers to our difficulties within. It is always an honor to be invited into the lives of those he meets. Brock has a strong interest in studying the ancient healing practices of indigenous cultures and the Shamans in those cultures.

Corinne Grant, MSW, LCSW

Corinne received her bachelors of social work from the University of Utah and her master of social work, emphasis in children and families, from the University of Southern California.  She has extensive experience working with teens and children, residential treatment, schools, and inpatient settings. Corinne has presented training on DBT, CBT, motivation interviewing, crisis management and short term therapy. She enjoys working with clients of all ages, mood disorders, personality disorders, grief and loss, depression, anxiety and women's issues. Corinne focuses on helping clients feel heard, find their voice, and gain insight to help them make positive changes in their lives. In her spare time she enjoys playing music, baking, and seeing movies.  

 

Dan Brennan, M.S.W., LCSW

Dan is originally from Long Island, New York, and came to live in Utah following his service in U.S. Army. He received his Masters degree in Social Work from the University of Utah. He also holds a Bachelors Degree from the University of Utah in Sociology with an emphasis in Criminology and Corrections. Dan has extensive work experience in foster care, aging, and troubled youth. He has presented training on Suicide Prevention. He was awarded Salt Lake City Exchange Club Award for 2010 – 2011. Dan enjoys working with people seeking to make changes in their lives. Dan strives to help people grow spiritually, heal inner wounds, and achieve greater harmony in their personal lives. He seeks to do so by creating an atmosphere where individuals feel safe to explore their personal concerns. When he isn’t busy working, you can find Dan with a fishing pole in his hand, biking around his neighborhood, or skiing Utah’s famous powder. 

Justin Olsen, LCMHC

Justin received his bachelors degree in Sociology from the University of Utah and his masters degree in Mental Health Counseling from the University of Phoenix. He has been a therapist for the last five years. Justin has extensive training working with individuals, couples and families. He spends a lot of time working with clients experiencing depression, anxiety, trauma, ADHD, substance abuse, and couples issues. Justin enjoys working with people that are motivated to be successful and just need a little extra help to reach their potential. He also enjoys helping couples learn to communicate more successfully. Justin has been on a local television show called The Place, talking about couples issues. In addition, he has been published by six different news paper agencies for articles that he wrote on self esteem issues, parenting tips, and ways to improve your marriage. In his spare time, he enjoys exercise, running triathlons, playing/watching sports, and spending time with his wife.

Katrina Miller, Ph.D., LMFT

Katrina H. Miller, Licensed Marriage & Family Therapist (LMFT), received her Ph.D. in Family & Human Development from Utah State University in 1997. When not at Blomquist Hale, she teaches Marriage and Family Therapy and other classes to student counselors at the University of Phoenix. Her research focuses on the techniques and attitudes needed for effective communication in intimate relationships. She enjoys doing therapy that helps individuals and families gain more insight into themselves and other people. She believes that insight can help people live more comfortably with themselves and joyfully with others. She has published articles on family and relationships in professional journals and consumer magazines. She was a featured expert on internet addiction on Fox TV and has presented at a national conference for counselors treating sexual offenders. 

 

Kerri Burns, CMHC

Kerri received her Master’s Degree in Mental Health Counseling from University of Phoenix, and a Bachelor’s Degree in Social Work with a minor in Psychology from Utah State University. She has extensive experience and training in marriage and family issues, depression, anxiety, grief and loss, and domestic violence. Kerri enjoys working with families and couples to identify and resolve communication issues, which in turn helps them strengthen their relationships. In her free time, she enjoys traveling anywhere there is a beach. She is an avid dog lover and currently a mom to 2 sons and 2 miniature schnauzers.

Kitt Curtis, LCMHC

Kitt received his Masters degree in Mental Health Counseling from the University of Phoenix, his Masters in Gerontology from the University of Utah, and his Bachelor’s Degree in Psychology from the University of Utah. He is a licensed Clinical Mental Health Counselor. He has extensive experience and expertise in working with substance abuse and dependence with both adults and youth, issues related to aging, loss and grief issues, marriage and family therapy, depression, anxiety, and general counseling. Kitt is the father of 5 children and has been married for 28 years. He enjoys spending time with his family. He loves boating, Lake Powell and Bear Lake. 

Nancy Paulsen, LCSW

Nancy received her Masters Degree in Social Work in 1996 from Brigham Young University.  She enjoys working with a diverse range of clients including individuals, couples and families. She has also worked with adolescents and children. Nancy has a broad range of experience and has worked in both inpatient and outpatient settings.  In her spare time she enjoys time with her family, music and traveling. She is a certified scuba diver and a fan of independent film.

Natalie Peacock, LCSW

Natalie received both Bachelors and Masters degrees in Social Work from the University of Utah, graduating in 2010. She has worked with children, adolescents, families, couples and individuals of all ages in a variety of different mental health settings including acute inpatient hospital, outpatient long-term therapy, and residential treatment. She particularly enjoys working with couples, empowerment through self-care, and the LGBT community. Since joining Blomquist Hale in early 2014, Natalie generally uses a person centered, strength based approach. Recognizing that each individual is complex and unique, she seeks to find solutions for each individual that will help them recognize they have the ability to create a happier healthier way of being.

She loves to spend time with family and her French Bulldog, Angus. She enjoys traveling and as a teen, had the opportunity to live in Japan. During her travels, she loves to try new foods and believes there is nothing better in the whole world than a great meal and a good book. She loves photography, gardening, cooking, and to baking in her free time.

Nikki Manson, LCSW

Nikki received her Masters Degree in Social Work with an emphasis in Mental Health from the University of Utah, and a Bachelors Degree in Social Work from Weber State University. She began working at Blomquist Hale in 2014 and is a Licensed Clinical Social Worker (LCSW). She has worked in both inpatient and outpatient clinical settings. Most recently she worked in a psychiatric unit specializing in crisis management and short-term therapy. Special interests include working with individual adults, couples therapy, substance abuse, chronic mental illness and life transitions. She enjoys working with clients within the therapeutic process to empower each to seek the changes they desire in their lives. In her free time she enjoys the outdoors and reading.

Olivia Verbeck, ACMHC, LSUDC

Olivia received her Master’s Degree in Clinical Mental Health Counseling from University of Phoenix, and a Bachelor’s Degree in Psychology Utah Valley University. She has extensive experience and training in substance abuse, domestic violence related issues, depression and anxiety, and anger management issues. Olivia gives clients space and acceptance to allow them to find what it is they want and need from their lives and experiences. She is interested in helping and understanding her clients better so that they in turn can understand themselves better. In her spare time, she enjoys fly fishing, being outdoors, learning, and reading.

Rick Ashby, LCSW

Rick is originally from the Chicago area of Illinois and came to Utah twenty-two years ago to be near family.  He received bachelors of science degrees in Psychology and Sociology from the University of Utah in 1998.  Soon after graduation he hired on with the Salt Lake County Sheriff's Office as a patrol deputy and continues to work as a police detective, now with the Unified Police Department.  He received his Master's degree in Social Work from the University of Utah in 2008.  Rick has clinical experience in inpatient and outpatient settings.  He has worked with a variety of issues including anxiety disorders, PTSD, stress and anger management issues, depression, various adjustment issues, and addiction.

 

Robert Hicks, CMHC

Rob received his BS in Psychology for BYU and his and MA in Professional Counseling from Westminster College. He has extensive experience working with addiction and family conflict, and enjoys working with motivated adults and adolescents who are looking to reduce and resolve conflict in their lives. Working towards change and improving personal peace and satisfaction is something that Rob focuses on with his clients. Rob and his wife operate a small publishing company as a hobby, creating and selling family board and card games. 

Sallie Weber, LCSW, M.Ed.

Sallie completed her Masters of Social Work at the University of Utah. She has a BA in English education and a Masters of Education from Northwest Nazarene University in Nampa, Idaho. She is also an elementary school counselor who works with whole classes, small groups and individuals. She also serves on a school district crisis team and has co-taught parenting classes. Sallie worked in a residential facility that provides care for seriously and persistently mentally ill adults and operates the SLC crisis hotline. Sallie often uses "The Work" of Byron Katie, a valuable adjunct to other therapeutic methods. She has worked with private therapy clients, presented workshops, taught college classes and taught junior high English. She loves reading and studying techniques for making life work and is inspired by helping people find their own wisdom and strength. 

Sammy Brown, CMHC

Suzanne received her Bachelors Degree from Montana State University, and her Masters degree in Professional Counseling from the University of Providence. She has more than 25 years of experience working with clients. Sammy enjoys working with individuals, families, and couples, and focuses on offering unconditional positive regard, acceptance of where they are in that moment, and building trust. In her spare time, she enjoys fly fishing, reading and playing card games with friends. 

Scott Roper, LMFT

Scott is a lifetime resident of Utah, completing his Bachelors in Psychology in 2005 and Masters in Marriage and Family Therapy in 2009 at Utah State University. He has worked with a variety of issues including depression, anxiety, impulse control problems, stress management, and various relational/couples issues. Scott enjoys hearing the experiences of the people that he works with and helping them to realize the strengths and skills that they already possess as well as working with them to build additional ones. Scott enjoys spending time with his family, watching, discussing, and playing sports, and collecting sneakers.

Steven Dahl, LCSW

Steven Dalh received his undergraduate degree in Phycology from Brigham Young University. He received his master’s degree in Social Work from Portland State University. Steven is a licensed social worker. He specializes in couples counseling, adoption issues, abuse recovery, anxiety and depression treatment and grief and loss work. Steven enjoys working with people because they are interesting and have unique stories and challenges. He finds it very rewarding to be a part of someone's success in improving the quality of their life. 

Steven R. Jones, LMFT

Steve received his BS in Behavioral Science and MA in Psychological Counseling from National University in Fresno, California. Along with providing his services to Blomquist Hale's clients and facilitating the relationship workshop, Steve is an adjunct professor at Utah Valley University, teaching Personality Theory, Counseling Techniques, and DSM-TR classes. He also runs a private practice for those who are not affiliated with Blomquist Hale Consulting. Steve's specialty is helping married couples, or partners planning on marriage, develop strong cohesive relationships by emphasizing personal development so each partner will be able to bring their best self to their marriage. Steve and his wife of over thirty years, Cary, have raised three children and enjoy spending time with family, especially their two grandsons.

 

Suzanne Sexton, LCSW

Suzanne received her Masters Degree from the University of Michigan, and has more than 20 years of experience working with clients dealing with depression, anxiety, grief, female issues and marital troubles.  Suzanne enjoys working with people to help them be all that they can be, and have the most fulfilling life possible. She empowers people to find and identify their strengths and creative solutions to challenging situations. In her spare time, Suzanne enjoys reading, music, meditation, cooking and being outdoors.

Ben Farmer, LMFT
U of U Clinical Supervisor

Ben Farmer received his Bachelors in Psychology and his Masters in Marriage and Family Therapy from Utah State University. He has worked with a variety of mental health issues including: depression, anxiety, behavioral problems, impulse control disorders, marital problems, general life stress, and adolescents with mild to severe behavioral challenges. Ben enjoys working with couples, families, individuals, and helping those in relationships work towards their goals. His strengths are in providing insight and understanding to the various situations that people face, encouraging hope, and helping clients utilize the skills they already possess and develop new strengths through the therapy process. His hobbies include mountain biking, trail running, OCR, watching movies, watching Youtube videos (there are some good ones!), listening to music, and striving to enjoy life.

Charlyn Raaberg, LMCHC

Charlyn received her Masters degree in Counseling Psychology from Pepperdine University in California and her BA from the University of California, Santa Barbara. She was originally trained and licensed in the state of CA as a Marriage and Family Therapist and is currently a Licensed Clinical Mental Health Counselor in the state of UT. Additionally, she has three years of advanced training in Gestalt Therapy. Charlyn has worked in a variety of mental health and wellness programs as well as in private practice. She has been a Psychotherapist, Wellness Presenter and Business Consultant for Blomquist Hale’s client companies and their employees since 2001.

Charlyn’s work with clients is based on the foundation that individuals are intricately linked to and influenced by their environments and that all people strive toward growth and balance. She believes in the wholeness and resilience of individuals and supports them in developing additional skills to broaden their engagement in life. Her areas of specialty are in relational growth, successfully managing life transitions, mindfulness for stress reduction, the treatment of grief and strategies for the treatment of depression and anxiety. In her free time Charlyn loves the adventure of travel and the outdoors as well as writing, creative pursuits, and the practice of mindfulness.

Judith Petersen, LCMHC

Judith attended the University of Utah and received her BA degree from San Jose State University in California.  After raising her children in Salt Lake, she returned to CA and earned a Master's degree from John F. Kennedy University in Orinda, California and held a California MFC license for a number of years.  She returned to her home state of Utah where she earned a Licensed Clinical Mental Health Counselor license. Judith enjoys working with individual, relationship, parenting, and GLBT issues.

Robyn Tate, LCSW

Robyn received her Bachelors degree in Psychology and her Masters degree in Social Work from the University of Utah. She has extensive experience in addiction therapy, as well as life transitions and relationships. Robyn believes that every human has strengths and weaknesses, and she admires anyone who is striving to improve themselves. Robyn is passionate about mental health and its influence on the quality of life.

Robyn Axtell, LCSW

Robyn received a Masters in Social Work from the University of Utah and is a Licensed Clinical Social Worker (LCSW). She also holds a B.A. in Human Resource Development and worked for over 10 years with major corporations conducting team building and management education programs.

Prior to joining Blomquist Hale and in her role as a therapist, she completed work and training in areas of trauma care, depression, anxiety, inpatient treatment, bipolar disorders, psychosis, substance abuse, grief counseling, crisis intervention, parenting skills, as well as marriage and family therapy. She likes helping families and couples strengthen relationships so that all family members feel accepted and safe within the family structure. She also enjoys working with individuals who struggle with mental health challenges such as depression, anxiety, mood disorders, unresolved trauma, and addictions. In doing so, her goal is to provide strategies, education, and insights that will move clients forward in a positive direction. When away from work, she enjoys traveling and reading.

 

Hollie Schelin, LCSW

Hollie received her Masters degree in Social Work from Brigham Young University and is an LCSW with 18 years of experience working with adolescents, individuals, couples and families. She has a positive, empowering approach in areas concerning anxiety, depression, abuse recovery, self-harm, anger, self-awareness, aging, grief and women's issues. She is keenly aware of the courage it takes for an individual to seek counseling and the trust development required to allow for change to take place. Her focus is to help individuals to find the peace, confidence and wholeness they each desire. Hollie is in love with life and the challenges it brings, and often seeks regeneration in the mountains sitting by a stream reading. 

Michael LePray, LCSW

Michael Lepray received his master’s degree in Social Work from Brigham Young University in 1993.He has published group therapy workbooks: "CBT for Depression and anxiety" and "Thinking Errors". Michael enjoys working with individuals, couples, and families and help them view their past success as a path way to overcome current obstacles. He is good at seeing the positive and potential in working with clients. In his spare time, Michael enjoys both international travel and international cuizine.

Pamela Anderson, LCMHC

Pamela received her Master of Mental Health Counseling degree from the University of Phoenix and a B.S. in Psychology from the University of Utah. Pamela is experienced in working with issues focusing on depression, anxiety, coping skills, relaxation techniques, assertiveness training, communication skills, mindfulness, stress management, grieving process, interpersonal emotional reactivity, improving self-concept and evaluating core beliefs. She enjoys the therapeutic process to assist clients to overcome personal obstacles. In her free time, Pamela loves teaching music to children, reading, camping and animals.

Natalie Jenson, LCSW
Ogden & Northern Utah Director

Natalie graduated from Columbia University with a Masters in Social Work in 2003. In addition, she has Bachelor’s degrees in Psychology and Sociology from the University of Utah. She has extensive experience in working with addiction and dependency issues and helping families navigate substance abuse recovery together. She has worked in a variety of settings including residential treatment for adolescents, outpatient mental health clinics, and substance abuse treatment programs. She has experience working with couples, families, and individuals with depression and anxiety and those experiencing difficult life events. Natalie enjoys teaching and has presented workshops and trainings at numerous companies. Natalie enjoys using a motivational interviewing and solution focused, positive psychology approach to help individuals improve their self-confidence and increase personal happiness. In her personal time, Natalie enjoys spending time with her family traveling, playing games, and going to concerts.

Cory Eddington, M.S., LMFT
Ogden & Northern Utah Clinical Supervisor

Cory received his Masters Degree in Marriage and Family Therapy from Utah State University. He also received a Bachelors degree in Psychology and Family Studies from Weber State University. He has served on the board, and been the Treasurer for the Utah Association of Marriage and Family Therapists as well as worked with the Weber County Coalition on Marriage teaching marriage enrichment classes. Cory is a Licensed Marriage and Family Therapist (LMFT) and has worked for Blomquist Hale since 2009. His experience spans many areas including both inpatient and outpatient settings. He has experience working with individuals, couples and families to improve some of life’s most difficult challenges such as depression, anxiety, marriage and family concerns, and self worth issues. He enjoys "working alongside my clients to help them see their own potential and strength, as well as to develop lasting skills to remain empowered in relationships and have peace in life and themselves." In his free time Cory enjoys water skiing, backpacking, and spending time with his wife and three children.

Ammon Fawson, LMFT

Ammon Fawson received his Masters of Marriage and Family Therapy from Utah State University. Coming to Blomquist Hale in 2015 after working in residential care for a number of years, he is experienced in treating anxiety, depression, parent-child difficulty, addiction, infidelity, couples’ issues, and grief and loss amongst others. He enjoys working with adolescents, adults, couples and families. Outside of work, he loves spending time with his wife and children, and is an amateur foodie. For Ammon, therapy is about hope. He works to build empathetic and understanding relationships with his clients, which allows them to find the hope, strength, and determination necessary to achieve great outcomes.

 

Catherine Callister, LCSW

Catherine received his Masters of Social Work from Walla Walla University. She is trained in EMDR and Dialectical Behavioral Therapy, and trauma focused CBT. Catherine enjoys working with all age groups and feels confident working with trauma survivors. Catherine believes in clients right to self-determination and enjoys empowering clients to overcome the obstacles in their life as they build on their strengths. In her spare time, Catherine enjoys hanging out with family and spending time with her two dogs.

 

Irene Parkhurst, LCSW

Irene earned her Masters Degree from the University of Utah.  Her focus has been family and marriage counseling, working with adolescents, and drug & alcohol abuse.  Irene enjoys working with people who are motivated to make changes in their lives. She offers a realistic approach based on lots of life experiences. Irene can connect quickly, and enjoys helping clients develop their life skills. In Irene's spare time, she enjoys reading, learning, gardening, and helping others. 

 

 

 

Linda Protzman, M.Ed., CMHC

Linda graduated from the University of Wisconsin, Milwaukee in Social Work and completed her Masters in Educational Psychology with an emphasis in Counseling at Brigham Young University with Phi Kappa Phi Honors. Linda is a Clinical Mental Health Counselor with over 30 years of counseling experience in both public and private counseling organizations. She was awarded 2007 Counseling Professional of the Year by the Utah Professional Mental Health Counselors Association.

A few of the populations Linda has served include developmental disabilities and mental retardation, youth corrections, children, teens, adults, couples and families of all types. She helps with a wide range of challenges such as PTSD, OCD, pornography addiction, stress, depression, anxiety, and interpersonal relationship facilitation. Linda served 8 years as a board member, 4 years of which she served as Chair of the Utah State Clinical Mental Health Counselor Board. In her free time, Linda enjoys new adventures such as swimming with sharks in the Dominican Republic, riding an elephant in Thailand and hiking new trails.

Tamra J. G. Poteet, ACMHC

Tami earned her Masters Degree in Clinical Mental Health Counseling from Capella University.  She is a certified Christian Life Coach and National Certified Counselor with the National Board of Certified Counselors. Tami has worked in private practice since 2013 in psychosocial rehabilitation and clinical therapy.  Tami is a Licensed Associate Clinical Mental Health Counselor.  Her focus has been anxiety, depression, and relationship issues.  Tami has experience working with trauma in youth and adults, overcoming negative thinking which contributes to anxiety and depression, and relationship coaching for enjoying more fulfilling relationships.  Tami enjoys helping women and men overcome their own self-doubt and empowering others to create a healthier way of life.  She currently facilitates women’s groups, including creating a life vision and relationship coaching.  Tami is the mother of two beautiful children.  She loves cooking, decorating, riding her ATV, and anything with music.

 

Brian Droubay, LCSW

Born and raised in Utah, Brian earned a Master of Social Work from the University of Utah in 2011. Prior to that, he graduated with a B.S. in psychology from Brigham Young University. Outside of clinical practice, he is currently pursuing a Ph.D. in social work. Brian has experience practicing psychotherapy in both inpatient and outpatient settings, with clients ranging from college students and staff to prison inmates suffering from severe mental illness. Before joining Blomquist Hale, Brian worked for over five with the Department of Corrections, most recently in the role of therapist supervisor. Given this experience, he is aware of the many stressors those working within law enforcement face day in and day out. His clinical interests additionally include anxiety disorders, depression, obsessive-compulsive disorders, faith crises, and the intersection of religion/spirituality and mental health. He has additionally worked extensively with persons struggling with substance abuse and unwanted pornography use. 


Brian has witnessed the significant pain individuals with mental health issues deal with and so has a great deal of empathy and compassion for his clients who are struggling. He firmly believes that people with emotional difficulties can find more peace in their lives, feel better generally, and progress toward their goals and aspirations. His approach to therapy is integrative of cognitive-behavioral and interpersonal modalities, with incorporation of acceptance and mindfulness practices. During his leisure time, Brian enjoys listening to music (especially jazz), golfing, running, watching Fawlty Towers reruns, and spending time with family.

Owner

David C Blomquist is a Licensed Clinical Social Worker, a Certified Personal Trainer and a Certfied Woman's Fitness Specialist.  He graduated with a Masters Degree from the University of Utah in 1978 and received Personal Training certification from the National Academy of Sports Medicine.  He is widely recognized as a pioneer in the fields of Employee Assistance and Health Coaching.  He is sought after as a speaker and presenter in these fields.

 

Brent Hale, Ph.D., LMCHC
Owner

Brent Hale received his Ph.D. in Psychology from Brigham Young University. He has also received a Masters degree in Marriage and Family Therapy from BYU as well as an undergraduate degree in Psychology from the University of Utah. As a consultant to business and industry, he specializes in the development of high performance teams, the assessment of organizational culture and strategies to accomplish organizational change projects. Brent is the Chairman of the Drug-Free Workplace Task Force for the State of Utah.

Sean Morris, LMFT
CEO

Sean received his Masters Degree in Marriage and Family Therapy from the University of Southern Mississippi. He also holds a Bachelors Degree in Family and Child Development from Weber State University. He is a Substance Abuse Professional (SAP). Substance abuse and mental health designated examiner. Sean is a motivated and strong business leader with an ability to build strong teams and is passionate about improvement for both individual and organizational success. He enjoys presenting on a variety of personal and organizational topics such as leadership, generational diversity, communication, and customer service. He is a Vistage member and Board Member for Lawyers Helping Lawyers. Sean enjoys snow skiing, running, and playing racquetball.

 

Theresa Jensen
Corporate Controller

Theresa earned her Masters Degree in Taxation from Weber State University along with her Bachelors in Accounting from the University of Phoenix. She is also a Certified QuickBooks Proadvisor and is currently working towards her CMA (Certified Managerial Accountant) license. Theresa has been with Blomquist Hale since November 2012 and serves as the Corporate Controller along with being a member of the Executive Leadership Team. Theresa brings the financial perspective to decision making company-wide, while also maintaining financial statements and monthly budgets and all things Accounting. In her free time she enjoys being as active as possible with running and cycling, loves to travel and explore great food everywhere.

Susie Evans
Director of Human Resources

Susie earned her B.S. in Business Administration from the University of Utah. She participates as a Utah County chapter member of the National Society of Human Resource Management (SHRM) and is currently working on her PHR certification. Susie has been with Blomquist Hale since March 22, 2011 along with being a member of the Executive Leadership Team. Her 27 years with Fortune 500 companies focusing on HR, management, and administration brings much experience to her current role. Susie enjoys creating a positive, diverse culture in business and with fellow associates. She is a California native who enjoys outdoor activities in Utah. She spends her spare time with her family and working with her grassroots animal rescue 501c3 and BLM wildland fire company.

Gia earned her B.S. in Family Services from Brigham Young University. She enjoys working with people. Her greatest satisfaction comes from reaching out to others and helping each other on this journey called life. In her free time, she loves going running, hiking, doing yoga, and spending time with her beautiful children.

Heather is responsible for the vision, creation, and execution of all marketing and graphic communications. Heather also manages website development and social media for Blomquist Hale. She has a passion for graphic design and photography and loves utilizing both in her job. In her free time, she enjoys spending time with her husband and three dogs.

Carolyn Memmott, CSW

Carolyn received her Master’s Degree in Social Work from University of Utah. She has extensive experience working with Dialectical Behavioral Therapy, teaching groups, and individual therapy. Carolyn enjoys working with clients who desire to better manage emotional stressors/responses, modify ineffective behaviors, improve interpersonal interactions, and who just want to move forward with greater awareness and confidence. Clients who work with Carolyn can expect a warm, non-judgmental atmosphere as well as a strong therapeutic alliance. In her spare time, she is passionate about meditation. She also finds joy in nature, especially through hiking. Her favorite place to hike is Moab, Utah. 

Christy Calderon, LCMHC

Christy received her Masters Degree from the New Mexico State University, and her Bachelors Degree from Texas Christian Universe. She has extensive experience in Cognitive Behavioral Therapy and Applied Behavior Analysis. She enjoys working with individuals who are dealing with depression, anxiety, and PTSD. She also has experience helping adults dealing with divorce, grief and loss, and retirement and life changing circumstances. Christy enjoys working with clients who are motivated to learn and grow to be their best self. She is a dog lover and proud owner of a Australian Shepard and Lab. In her spare time, she enjoys walking and hiking with her four-legged fur babies.
